45-50 minute session – $150.00


Insurance

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your insurance carrier to verify how your plan covers you for psychotherapy services.


Therapy-related expenses are the client’s responsibility, regardless of insurance coverage. While we verify your benefits with your insurance company as a courtesy, copay or coinsurance info provided by us is not a guarantee of insurance coverage or payment. Clients are responsible for knowing the stipulations of their insurance policy. If for some reason, your insurance company fails to pay for services provided and/or you are not eligible at the time the services are rendered, the client is still responsible for payment.

You might ask these questions of your insurance carrier to help determine your benefits:

  • Does my health insurance plan include mental health benefits?

  • Do I have a deductible? If so, what is it and have I met it yet?

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  • Do I need written approval from my primary care physician in order for services to be covered?

Payment

Cash, checks, and major credit cards are accepted as forms of payment. Payment is due at the start of session.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 48 hours beforehand. Otherwise, you may be charged for the full rate of the session.

Medical expenses are the patient’s responsibility regardless of insurance coverage. While we verify your benefits with your insurance company as a courtesy, a copay or coinsurance info provided by us is not a guarantee of insurance coverage or payment. Patients are responsible for knowing the stipulations of their insurance policy. If for some reason your insurance company fails to pay for services rendered and/or you are not eligible at the time the services are rendered, the patient is still responsible for payment.


You also agree to take full responsibility for the entire amount due for any and all services rendered that are not covered by your insurance carrier. Notify the office for any changes of insurance or demographics information. You authorize your insurance plans to pay directly to Cameron Murdock Group, LLC for the services provided.
